SMAS Fixation

(Preventing Ectropion Through Deep Structural Support)

SMAS fixation is a critical technique in revision lower eyelid surgery, particularly in patients at risk for ectropion or recurrent eyelid instability.
Rather than relying on skin tightening or superficial tension, Dr. Ahn restores lower eyelid support by anchoring the eyelid complex to the Superficial Musculoaponeurotic System (SMAS)—a deeper, more stable structural layer.

In revision cases, ectropion often develops when downward forces are concentrated at the eyelid margin following excessive skin removal or scar contracture.
SMAS fixation redistributes this tension away from the eyelid edge, allowing the lower eyelid to maintain a natural position without excessive traction.
